Community Needs Assessment
The Community Health Needs Assessment (CHNA) process generally takes place every three years. It surveys various members of the community and determines the most pressing health needs the community is facing. It is a cooperative approach by Fauquier Health, the PATH Foundation, and the Virginia Department of Health to understand the needs of residents in Fauquier and Rappahannock counties.
