Please choose your preferred hours below and our team will work to schedule you based on your selection. Your selection may not be guarantee based on demand.  To be eligible, you must have an order from your physician

You may also self-request by following criteria below:

1. You must be 40 years of age or older.
2. You must have had an appointment with your primary care provider within the last 18 months.
3. Your last mammogram must have been performed at least 365 days ago.
4. You cannot have any current breast symptoms such as pain, nipple discharge, lumps, etc. 
5. You cannot have a personal history of breast cancer within the last 5 years.
